Bariatric Surgery Evaluations
Many bariatric surgery programs require a clinical evaluation as part of the approval process. This assessment helps identify strengths, support systems, and factors that may impact long-term success following surgery.
Our bariatric evaluations include:
• Comprehensive clinical interview
• Review of mental health and treatment history
• Assessment of emotional and behavioral factors related to surgery readiness
• Documentation for your surgical team, when appropriate
• Virtual appointments available throughout Texas
Your Investment

Your fee covers the clinical assessment, evaluation, and written documentation process. Clinical recommendations are based on your evaluation findings and the requirements of your bariatric surgery program. Fees are non-refundable once the assessment has been completed.
Payment is due at the time of scheduling unless other arrangements have been made. HSA and FSA cards may be accepted depending on your plan.
Emotional Support Animal Evaluations
An Emotional Support Animal may provide meaningful support for individuals experiencing mental or emotional health challenges. Our evaluation process is designed to determine whether an ESA is clinically appropriate based on your individual needs and circumstances.
Our ESA evaluations include:
• Comprehensive clinical interview
• Review of current symptoms and functioning
• Assessment of disability-related need
• Clinical recommendations and supporting documentation, when appropriate
• Telehealth appointments available throughout Texas

Your fee covers the clinical evaluation, review of supporting information, and documentation process. Payment does not guarantee approval or the issuance of an ESA letter. Recommendations and documentation are provided only when supported by clinical findings. Fees are non-refundable.
